Introduction

Today, we will be sharing a simple guide on how to reconstitute oral antibiotic suspension.

NOTE: It is important to check the product leaflet for specific storage and stability instructions post reconstitution as these may vary depending on the product.

The standard procedure should include:

  • Shake the bottle to loosen the content. All the granules should be free-flowing in the bottle (i.e. nothing sticks on the bottom of the bottle)
  • Add an amount of drinking water and shake well till all granules are dissolved.
  • Top up with drinking water to the maximum mark.
  • It is now ready to serve and remember to shake well before each dosing.

For Augmentin oral suspension, to reconstitute the dry powder to form an oral suspension, you can either

  • Add a specific volume of water and shake well; OR
    • Augmentin 228 mg/5 ml, 70 ml = To add 64 ml water
    • Augmentin 228 mg/5 ml, 140 ml = To add 128 ml water
    • Augmentin 457 mg/5 ml, 35 ml = To add 31 ml water
    • Augmentin 457 mg/5 ml, 70 ml = To add 62 ml water
    • Augmentin 457 mg/5 ml, 140 ml = To add 124 ml water

  • Fill the bottle with water to just below the mark on bottle label. Invert and shake well, then top up with water to the mark. Invert and shake again. Allow to stand for 5 minutes to ensure full dispersion.

Preparation of azithromycin 200 mg/5 ml suspension

  • Shake the dry powder loose.
  • For 15 ml (600 mg) bottle: add 8 ml water to the powder.
  • Shake well until a homogenous suspension is achieved.

The key difference in the reconstitution procedure: The total amount of cold water to be added for reconstitution is the amount of water up to volume line on the cup provided (20 ml).

Hence, the instructions are as below:

  • Shake bottle well to loosen granules.
  • Fill the measuring cup to the line with cold water (NOT hot or warm liquids)
  • Pour the total amount of cold bottle into the bottle in one go and replace cap.
  • Allow the bottle to stand to allow the water to full soak through the granules; this should take about one minute.
  • Invert the bottle and shake well for at least 15 seconds until all the granules have mixed with the water.
  • Turn the bottle into an upright position and shake well for at least one minute until all the granules have blended with the water.
  • Store the cefuroxime axetil suspension immediately at between 2 and 8°C (do not freeze) and let it rest for at least one hour before taking the first dose.
  • Always shake the bottle well before taking the medication.

Powder for Oral Azithromycin Suspension (Brand: Zithromax)

  • Tap the bottle to loosen the powder.
  • Add water to the bottle.
    • To the 600-mg bottle, add 9 ml of water;
    • To the 900-mg bottle, add 12 ml of water;
    • To the 1200-mg bottle, add 15 ml of water;
    • To the 1500-mg bottle, add 19 ml of water.
  • Shake well.
  • Shake immediately prior to use.
